Issuing a travel document for return to the Republic of Uzbekistan.
The Travel document for return to Uzbekistan is issued by the Consular Section of the Embassy to citizens of Uzbekistan and stateless persons permanently residing in Uzbekistan in cases of loss, damage, expiration or use of the pages of an Uzbek citizen's passport or a stateless person's travel document, or if the person has not previously been documented by a passport.
In order to obtain the certificate of return to Uzbekistan, the applicant must appear in person at the Embassy for a consular interview and submit the following documents:
1. Antrag an den Konsul der Botschaft der Republik Usbekistan in Deutschland.
2. The completed Application form is in two copies. The application form is filled in electronically on the website of interactive consular services of the Ministry of Foreign Affairs of the Republic of Uzbekistan - consul.mfa.uz or consulate.mfa.uz. A fully filled out questionnaire with detailed data in each column, uploaded online with a digital photo, is considered valid. After filling in, the questionnaire should be printed on a separate sheet with a bar code in the upper left corner.
3. Photos 3.5 x 4.5 cm (2 pcs).
4. The original and a copy of the passport, in case of damage, expiration of the passport or the use of all pages.
5. Certificate of passport loss from the police (in case of passport loss). The certificate should be certified by the competent authority of the host country and translated into Uzbek or Russian by a sworn translator. Together with the original, a copy of this document with translation is also provided.
6. If a passport is lost, a copy of the lost passport or identity card and/or other document proving the identity of the applicant issued by the authorized bodies of the Republic of Uzbekistan should be provided as far as possible.
7. The original and a copy of the birth certificate. Birth certificates may be available:
a) Birth certificate (metric) issued by the civil registry offices in Uzbekistan or in Uzbek consular offices in foreign countries;
b) Birth certificate issued by the civil registry offices in Germany. This document should be legalized in the embassy in accordance with the established procedure. The original German Geburtsurkunde must be presented to the Landgericht. Then, together with a copy of the original, you must provide a copy of the paid receipt to the Embassy - 25 euros for citizens of RU and 65 euros for foreigners. Only then will the document issued by the German institutions be recognized by the Uzbek authorities;
c) Birth certificate issued by the registry offices of the Czech Republic (Rodny list), Switzerland and Sweden (Personbevis). Due to the mutual recognition under the Hague Legalization Agreement, this document must be apostilled (stamped with an Apostille) by the relevant authorities and translated into Russian and Uzbek by a sworn translator. Only after an apostille and a sworn translation of a foreign document by a sworn translator, the document issued by Czech, Swiss and Swedish institutions will be recognized by the Uzbek authorities.
8. Original and copies of passports (non-biometric, biometric and/or biometric for traveling abroad) of parents or other legal representatives of the minor.
9. A copy of the parent's marriage certificate, if available.
10. A receipt for the payment of consular fee in the amount of EUR 60, for children under 16 - EUR 10.
The certificate shall be issued only upon receipt of the relevant confirmation by the competent authorities of the Republic of Uzbekistan.
The certificate is issued for the period from one to three months. The validity period of the certificate is not renewable.
The certificate is issued for each person separately. Children under age may not be added to the certificate.
In case the certificate is issued due to expiration, spoilage or waste of pages of the identity document, the said document shall be submitted to the Embassy.
Upon arrival in the Republic of Uzbekistan, the applicant must submit the certificate to the internal affairs authorities of the place of residence no later than before the expiration of the certificate to obtain a new passport.
In the following cases, the applicant may be refused a certificate of return to Uzbekistan:
- Reporting knowingly false information about himself or herself or a false loss of a travel document;
- Failure to meet the requirements (incomplete list, incorrect filling in).
Payment for all consular activities at the embassy is made:
- by bank payment card during the reception at the Embassy;
- by transfer before the reception at the Embassy (Überweisungsbeleg), the copy of the receipt should be attached to the documents.
